High Sky LIFE Program

LIFE Aftercare is a program designed to assist youth ages 17-21 in making the transition out of foster care to independent living. While PAL teaches youth about life skills, the LIFE Aftercare program assists youth in completing activities. Many youth leaving foster care may not have family to give them guidance and support as they move toward becoming an adult. The LIFE Case Manager is available 24-hours a day to provide emotional support and guidance, along with help finding housing and jobs, providing emergency funds for food, medical expenses, utilities, etc. Aftercare is voluntary for all youth and is available in every county in Texas.

The LIFE Aftercare program provides intense and proactive case management. Case managers develop individualized transition plans with each youth outlining their goals. Case managers will match the intensity of each youth and provide as much assistance as possible, provided the youth is working the program. Aftercare Case Managers take a proactive approach with clients and try to prepare them to successfully handle future situations, rather than waiting for crises to occur. The program is designed to be a tool to help youth learn how to be successful in achieving independence.

LIFE Aftercare can assist in the following areas:

  • Direct youth to appropriate community resources
  • Inform youth about benefits and how to access them
  • Assistance with completing applications (housing, jobs, college, vocational programs, etc.)
  • Assistance with accessing funds for higher education
  • Teaching life skills and preparing youth for adulthood
  • Assisting youth with problem solving and self-improvement
  • Reminding youth about important deadlines (i.e. filing taxes, applying for college, etc.)
  • Providing emergency funds for room and board and other unexpected financial hardships
